Child Find Notice: Overview

The School District of Marshfield is committed to locating, identifying, and evaluating all children with disabilities residing in the district—including those in private schools, homeschooled, or homeless—regardless of the severity of the disability.

Referral and Evaluation Process

A referral is a written request for evaluation. Once received, the district assigns an Individualized Education Program (IEP) team to determine whether the child has a disability and needs special education services.

Who Can Make a Referral?

  • Physicians, nurses, psychologists, social workers, or social agency administrators (required to refer)
  • Parents, guardians, and others with concern (may refer)

Referrers must notify parents before submitting the referral.

How to Make a Referral

Referrals must be in writing and include the reason for concern.

Early Childhood Screening (Ages 2y9m – 5y)

Families with children not yet in 5K can request developmental screenings using the Ages & Stages Questionnaire (ASQ-3). This screening evaluates:

  • Communication
  • Gross & fine motor skills
  • Problem-solving
  • Personal-social development

4K students are screened automatically. Families with younger children can request an ASQ-3 by contacting:

Pupil Records & Confidentiality

The district collects and maintains records as part of Child Find and special education evaluation. These are protected under FERPA, IDEA, and Wisconsin Statutes §118.125.

Types of Pupil Records

  • Progress Records: Grades, courses, attendance, immunizations, extracurriculars (kept 5 years after enrollment ends)
  • Behavioral Records: Psychological tests, evaluations, behavioral reports (kept 1 year post-enrollment unless extended)
  • Directory Data: Name, address, phone, birthdate, photos, awards, activities (parents may opt out of disclosure)
  • Physical Health Records: Screenings, emergency cards, athletic health forms, and basic medical info

Parent and Student Rights

Under FERPA and IDEA, parents and eligible students (age 18+) have the right to:

  • Inspect and review records within 45 days of request
  • Request corrections to inaccurate or misleading information
  • Consent (or deny consent) for disclosure of personal information, except where allowed by law
  • File a complaint with the U.S. Department of Education:

Child Find Notice

The school district must locate, identify, and evaluate all children with disabilities, including children with disabilities attending private schools in the school district, regardless of the severity of their disabilities. The school district has a special education screening process to locate and screen all children with suspected disabilities who have not graduated from high school.
